Professional Reference Request Form

$20.00

It is recommended that candidates who are interviewing for a position provide references, information and authorize the organization to contact the reference(s). Once the references have been contacted and the information documented, whether or not the candidate is hired the candidate resume, application form, interview notes and reference information needs to be filed and stored for a given period of time depending on your state laws. For those candidates that are hired, it is recommended that the resumes, application form, interview notes and reference information be filed and stored in the employee’s personnel file based on your state’s requirements. Please contact your human resource department, labor attorney or United Physician Services for your state’s requirements.
