**Section 3 of this document provides an example of how to calculate your fees as a percentage of the AHCCCS/Medicaid and Medicare fee schedule. It is recommended that all services (CPT) codes be set at anywhere between 1.5% to 300% of the AHCCCS/Medicaid or Medicare fee schedule based on type of practice and location.
